To Our Patients:
Welcome to NEW IMAGE PLASTIC SURGERY, LLC, which is owned by Pamela S. Henderson, M.D., P.C. and Steven H. Wiener, M.D., P.C.
Your Surgeon/Physician: We would like you to know that Dr. Henderson is board certified by the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology-Head and Neck Surgery and is licensed in the State of Arizona. She has been in practice since 1994 and attended medical school at the University of Arizona College of Medicine in Tucson, AZ, residency training in otolaryngology at Oregon Health Sciences University in Portland, OR and a fellowship in facial plastic and reconstructive surgery at the Facial Plastic Surgery Center in St. Louis, MO. You may request a C.V. which we keep on file. Her training is extensive in the field of facial plastic surgery. Should you choose to have surgery, you will have the option of having the procedure performed at the in-office surgical suite or the hospital depending on individual circumstances and the procedure being performed. At this organization, Dr. Henderson will be the only one performing your surgery if she is your physician.
Your Anesthesia Provider: Additionally, this organization utilizes Certified Registered Nurse Anesthetists (CRNA’s) as credentialed anesthesia providers, with many years of experience and training licensed in the State of Arizona.
The Team: Our team is made up of competent individuals that will assist in providing safe patient care including trained medical assistants, surgical technicians, and a licensed aesthetician in the State of Arizona.
Independent Business Contractors: Within the office suite, we have two independent contractors which are separate business entities from New Image Plastic Surgery and not associated with our accreditation: New Results Medical Weight Loss and Marilyn’s Makeup Magic. Please address questions or concerns about those entities directly to them.
Should you have a problem: Please be advised that if you have a grievance or concern the following mechanism exists: Ask for the grievance form from the receptionist. Or you may call the accrediting organization that oversees our compliance with standards of care: The Accreditation Association for Ambulatory Health Care, Inc at 847- 853-6060.
Make a suggestion: If you have a suggestion, please place this is writing and hand it to the receptionist, place it in the suggestion box in the waiting room or mail it to the office.
Play a part in your care: We encourage all patients to be actively involved in their care, so please speak up and ask questions of anyone in this organization.
Additionally, please be advised that this organization does not recognize Do Not Resuscitate orders or Living Wills. If you have any questions, please see the receptionist.
